" Dialup Service - Signup Form"

Thank you for choosing CVC Internet as your Internet provider.  To activate a dial-up account, please complete the following information below(*required information). Once the form is complete, click submit or print a copy of the form and fax it to 541-884-8943, or bring it by our office located at 2250 South 6th Street, Klamath Falls, Oregon.

You will be notified when your account is active and will be instructed what to do for setup.  If you do not receive a confirmation within 24 hours, please contact the office at office@cvc.net or call (541) 884-5488.

Please Note:  If you sign up for service after the tenth of the month, please contact the office for the first month's peroration fee by calling (541) 884-5488 or by e-mailing office@cvc.net. For accounts paid by check or money order, service will be activated once the first service payment is received by CVC.  All Dial-up Services are available in the Klamath County, Oregon region only.
