Dial-up Internet Sign-up Form

Thank you for choosing CVC Internet, LLC as your Internet Service Provider.  To activate a Dial-up Internet account, please complete the service form and make sure to complete the required information fields.  Please Note:  If you sign up for service after the tenth of the month, please contact the office for the first month's peroration fee by calling (541) 884-5488 or by e-mailing office@cvc.net. For accounts paid by check or money order, service will be activated once the first service payment is received by CVC Internet, LLC.  Before signing up for service, please review our System Requirements and Conditions of use.
